Clinical Director - TCM
Our client is a growth-stage healthcare services and technology company building a post-discharge service line for Medicare Advantage, Medicaid, and Exchange members. Lead the clinical operations that turn timely outreach, in-home and virtual care, and practical follow-up into fewer avoidable readmissions. You will work with an existing team and report to the Chief Clinical Officer.
Full-time | Remote, U.S. | $150,000–$180,000 base + performance bonus, equity, and benefits. Equity terms will be discussed during the process.
